Billing & Insurance

Insurance and Payment Information

We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.

We require that services be paid in full on the day received, unless other arrangements are made ahead of time. Grand Traverse Surgery works closely with several insurance carriers, so we can bill these carriers for you. However, any co-pay, deductible, or co-insurance amount must be paid on the date of service. You will be expected to present your insurance card at each visit. Please direct any questions about your coverage or billing statement to (231) 346-4000.

Appointments

Before your appointment with us, quickly and conveniently access patient forms from our practice.

We understand that your schedule may change and you may need to cancel an appointment. Please inform us 24 hours in advance by calling (231) 346-4000.

If you are a new patient or have an appointment for an annual physical, we ask that you arrive 15 minutes before your scheduled time to fill out a patient information sheet. Also, please bring your medications and insurance card so we may review them with you.

Test Results

Please allow seven business days before calling our office for test results.
